About Us
Built on purpose,
driven by care
Flourish Health is a values-driven community allied health provider delivering in-home occupational therapy across Melbourne. Our AHPRA-registered clinicians are motivated by one thing: helping people live more independently in their own homes.
It started with a question no one was asking
Hayley had spent years working as an occupational therapist in clinics. The assessments were thorough, the interventions well-planned. But something wasn't translating. Participants would practise skills in a clinic kitchen, then go home and struggle in their own.
The question was simple: why are we assessing people in environments that don't reflect their actual lives?
That question became Flourish Health. Hayley co-founded a practice built entirely around in-home therapy, because the environment where someone lives is not a detail - it is the whole picture. A grab rail recommendation is different when you have seen the actual hallway. A morning routine plan works better when your therapist knows the real layout of your kitchen.
Person-centred care that listens first
Everything we do is grounded in three clinical principles. We draw on established occupational therapy frameworks including CMOP-E, MOHO, and Occupational Performance Coaching, and we measure outcomes using validated tools like the COPM and Goal Attainment Scaling.
You are the expert on your own life. Our role is to bring clinical knowledge and practical skills, then work alongside you to find solutions that match your priorities.
- Evidence-based assessments informed by current research
- Person-centred goal setting - your goals drive your therapy
- Occupation-focused - enabling the daily activities that matter to you
- In-home delivery so recommendations match your real environment
- Family and carer involvement in the therapy process
Our values make us who we are
These aren't words on a wall. They shape every home visit, every assessment, and every conversation we have with you and your family.
Your Life, Your Way
We exist to help you do more for yourself. Every session is aimed at building your capacity and confidence to live independently, on your terms.
We See You, Not a Diagnosis
You deserve respect, choice, and a voice in every decision. We practise with dignity of risk, supporting informed choices even when the path is uncertain.
We Come to You
Therapy should happen where life happens. Our in-home model removes the barriers that clinics create, whether that is transport, mobility, or anxiety.
Your Goals Run the Show
We listen first, then build a plan around what matters to you. Cookie-cutter programs do not work, and we refuse to use them.
Straight Talk, Always
No jargon, no runaround. We explain what we are doing, why we are doing it, and what to expect. You are part of every conversation, not an afterthought.
We Show Up for You
Our therapists are invested in your progress, your setbacks, and your wins. This is not just a job, it is a commitment.
