Will making a complaint affect my services?

No. Making a complaint will never affect the services you receive from Flourish Health. We treat all feedback confidentially and use it to improve our care. You have the right to raise a concern at any time.

Can I provide feedback anonymously?

Yes. You can submit anonymous feedback through the form on this page by leaving the name and contact fields blank. However, if you would like us to follow up or resolve a specific concern, we will need your contact details to respond.

What if I am not satisfied with how my complaint is handled?

If you are not happy with our response, you can escalate your concern to an external body. The NDIS Quality and Safeguards Commission (1800 035 544), AHPRA (1300 419 495), or the Health Complaints Commissioner Victoria (1300 582 113) can all receive complaints independently.

Can someone else make a complaint on my behalf?

Yes. A family member, carer, support coordinator, plan manager, or advocate can submit feedback or make a complaint on your behalf. You can also contact the NDIS Commission directly if you prefer an independent process.
